Tax Exempt - Hotel/Lodging

Many states extend exemption to government employees for hotel stays.  In order to receive the benefit, the traveler must provide the tax exempt form.  For convenience, I have created a standard form that covers those states participating in this program.  The form is uploaded under 'Travel Resources'.  Please include/provide the form, as this is a cost savings to the government.
